Dear Sir or Madam,

I am a student of "Bachelor of Engineering" with Computers as a major in our college. My intent of writing this letter is to request for issuing a new student identification card.

On 14th of March in the morning, I met with a car accident while traveling for the institution, in that accident I got several injuries and a fracture in my left arm, later some people dropped me to the hospital and doctor suggested me proper bed rest for a month. All things happen so sudden that I lost my student identification card somewhere on road and after my all worthless efforts, I was not able to collect it back.

Now, I am at least able to attend the college but, I require to show the proof every now and then to be the scholar of college for security reasons, in addition as exams are near I am also not able to get reference books or other study material without it from the library. To resolve the issue, it is my humble request to commence the process of issuing a new student identification card for me. My previous student number is 'BT2019008007'.
