RE: Mr Gerald Baker,79 years
I am writing this letter to inform you regarding Ms.samantha bruin who requires further assistance and care following total hip replacement surgery.
Ms.bean was admitted on 3/7/2018 with the complaints of difficulty in moving and climbing the stairs . further evaluation conformed osteoarthritis and the surgery was done the very next day. Postoperatively, her haemoglobin level dropped to 72g/dl and was managed by blood transfusion. She commenced on aspitin 100mg,ramipril 5mg and pandeine.
Her past medical history reveals that she is a know case od hypertension and osteoarthritis. Socially, she is a widower and lives in a nursing home for the past 4 years. She has no children but her sister, Ms dowm manson visits her regularly. she is an aged pensioner.
As per the discharge plan, it would be greatly appreciated if you could dress the surgery incision site .please encourage him to do stretching and strengthening exercises. Also , an occupational therapy would be beneficial for him. Kindly note his blood haemoglobin level for anaemia . of note, his staples has to be removed in two weeks time and kindly follow up FBC and UEC test .
