Dear Sir or Madame,
I am writing to let you know that I have just received my letter that was supposed to include my new VISA card. Unfortunately, I found out that my new card was not enclosed when I opened my envelope. Plus, I also made a call to your hotline, but there was no answer.
I cannot do the acknowledgement the receipt of my card as your requirement since I have not received it yet. Plus, I am also worried about whether I have to pay for any purchases that I actually have not used it because somehow my card was delivered to a wrong person or got lost in its way.
In the meantime, please do lock my card so no one else can use it. In addition, would you please send me a new card as soon as possible in more secure method?
