Dear Sir or Madam,
I would like to bring to your notice a recent request that I had made to your bank branch in MG road. I had asked the bank to provide me an expression debit card. The manager had approved the same and I was supposed to receive my card by last week. However, I have not received the card yet.
Firstly, let me provide you with the details and purpose of my writing. I am Madhulika Banerji and have saving bank account with your branch for almost a decade now. The customer id issued to me is GHYRHT and the account number is 855698566. The reason I am writing this letter is that though the bank is claiming to have sent the card and new pin details, I have only received the pin details and not the card. Since I did not receive the card, I, therefore, cannot give the acknowledgment receipt of the card.
Adding the above point, I am absolutely sure that my card has not been stolen as the envelope was sealed when I had found it in my mailbox. My concern, however, is that the employee forgot to place the card inside the envelope. This is a very horrible service and would definitely spoil the reputation of the bank if it continues.
In the end, all I request from you is to investigate this matter as soon as possible and also issue a new debit card to my address. I am hoping that strict action will be taken so that mistake is not repeated ever again. I will be providing my acknowledgment of the card received only after I get the card.
